The WellSky CarePort Referral Intake solution leverages AI-powered document summaries to power smarter care transitions for post-acute providers

Building on the proven CarePort and Curaspan platforms, this advanced solution combines trusted features with modern Artificial Intelligence (AI)-driven capabilities designed to streamline referral intake workflows and generate document summaries, enabling providers to respond to referrals faster and more efficiently.

Timely responses to patient referrals are critical for post-acute care providers to secure business, ensure smooth transitions, strengthen referral partnerships, and improve patient outcomes. The new WellSky CarePort Referral Intake solution incorporates WellSky Summarize, a tool powered by WellSky SkySense AI™, which scans referral packets, extracts key patient information, and generates concise, easy-to-read summaries within seconds. This allows providers to quickly evaluate patient needs and respond to referrals with greater speed.

Providers gain a centralized view of referral data, enabling real-time monitoring of referral volume, response times, and patient placements. Advanced analytics provide insights into engagement trends and conversion rates across referral partners, helping organizations identify patterns, optimize strategies, and strengthen their role as referral partners.
