WellSky® Launches AI Ambient Listening & Transcription in EHR

WellSky, a global leader in health and community care technology and services, has announced the integration of advanced Artificial Intelligence (AI) capabilities into its flagship electronic health record (EHR) platform, WellSky Home Health, powered by SkySense AI. This update introduces WellSky Scribe, an ambient listening and transcription tool designed to streamline clinical documentation within the EHR workflow.

The SkySense AI-powered WellSky Home Health platform offers a growing suite of AI-driven solutions aimed at improving operational efficiency for healthcare organizations. WellSky Scribe, the latest addition, captures real-time conversations between providers and patients, automatically generating clinical notes within the EHR. This allows clinicians to focus on patient care during home health visits, enhancing both clinician and patient experience while addressing challenges in staff retention and recruitment.

Clinicians often navigate over 100 data fields during a single patient start-of-care visit. With multiple visits each day, much of this documentation traditionally occurs outside working hours, increasing the risk of burnout, errors, and costly overtime. WellSky Scribe reduces this burden by automatically populating documentation into the EHR, allowing clinicians to dedicate their attention to patient assessment and care.

“The demands on home health clinicians to assess patients, provide care, and complete extensive documentation has led to burnout and workload issues, which contribute to the ongoing staffing challenges within the industry,” said Wes Little, executive vice president, analytics and AI, at WellSky. “WellSky Scribe is a great example of how we are responsibly introducing AI solutions that help solve real problems and create immediate value for our clients.”

During a visit, clinicians activate WellSky Scribe, which identifies key terms from the audible assessment and populates the relevant EHR forms and fields. Afterward, clinicians review and confirm the accuracy of the auto-populated data, submitting it within minutes and saving substantial documentation time.

In December, WellSky will roll out the WellSky Intelligent Language Assistant (WILA), a conversational voice assistant designed to work with WellSky Scribe to complete any remaining OASIS documentation. Clinicians can interact with WILA post-visit, which guides them through unanswered questions and populates responses directly into the EHR. Together, these tools support faster, more accurate documentation, with future expansions planned beyond the start-of-care visit.

Building on previous AI successes, WellSky’s WellSky Extract, launched in January 2025, automatically compiles patient medication information from multiple sources, including referral documents, handwritten lists, and medication label photos, and populates it into the EHR. Users have reported up to an 80% reduction in medication reconciliation time, underscoring WellSky’s commitment to leveraging AI to enhance home health care efficiency and accuracy.
